Insurance Verification and Prior Authorization
Before your initial TMS appointment, please provide your complete insurance information.
Our office will work with your insurance provider to obtain any required prior authorization and estimate your out-of-pocket costs, including co-pays and deductibles.
If your insurance coverage changes, or if you plan to change insurance plans, please notify our office as soon as possible to avoid delays in your treatment.
Payment Information

Co-pays are due at the time of each visit.
