FIRST VISIT INSTRUCTIONS


PLEASE COMPLETE THESE STEPS BEFORE YOUR FIRST VISIT:

This is a step by step guide to follow prior to your first appointment. 

Note: Treatment consent forms are electronic only and will be required prior to your first visit.

 

1.  Contact your insurance company to determine if my services are covered by your insurance provider.  Please confirm that my services are covered by your insurance plan prior to your first visit. If my services are not covered, or if you have not met your deductible, you will be financially responsible for those services.

 

2.  Contact me by phone at (206)-602-9162 to determine if my services are a good fit for you.  After setting up an appointment with me I will send an invitation via e-mail to the patient portal.  I will also send specific instructions on how to complete the onboarding process.  At the portal you will complete the new patient consent forms.

Those steps are as follows:

  1. Follow the invite link to the onPatient portal

  2. Sign in

  3. Find your next scheduled appointment

  4. Click “Check in”

  5. Scroll down to find the consent forms, click the name of the consent document ( blue color)

  6. Scroll down click “I’ve read this document.”

  7. Sign and save signature.

  8. Scroll down click “I’m done.”

After this initial setup you will be able to log into the patient portal.  The portal is usually used for the initial onboarding process only but if you need to access this site again, you can find it by clicking the ORANGE LETTERING at the top right of this website, or at onPatient.com at any time after you have set up your account.  Note:  Your appointment may be canceled if you do not complete the initial onboarding process.

3.  Prior to your appointment write down symptoms (e.g., sleep changes, irritability, shortness of breath, etc) you have been experiencing.  Make note of the medications you have tried in the past, and if possible medications that related family members have tried (both helpful and unhelpful). 

4.  Be sure to bring your driver's license and insurance card to your first visit.
